REGISTRATION
All participants must submit 100% of the fee with their application to reserve a spot for each session. There are no refunds on payments. All league & clinic registrations are non-refundable. Any clinics canceled due to weather, rink closures, anything out of ENEH control, etc. participants will be issued a credit for future use. A confirmation letter will be sent upon registration.
AGREEMENT & WAIVER
Participant(and particpant’s Parent(s)/Guardian(s))acknowledges, understands and agrees that the sport of Ice hockey can be dangerous and involve risks of injury, and assumes all risks relating to ice hockey and any ENEH activities. This further includes risks of sickness arising from any and all communicable disease, germs, bacteria, viruses or infections, including without limitation the virus responsible for COVID-19 and other similar viruses or infections like COVID-19 whether now known or later discovered.
I agree that I shall provide health insurance to cover any personal injury and property damage sustained by my child/guardian while participating in any activities of or while on the premises of Erik Nates Euro Hockey, Inc. I hereby release, discharge Erik Nates Euro Hockey, Inc., Erik Nates and employed, from all damages, suit, causes of action, or liabilities for personal injuries and or property damages which may arise as a result of any such accident or loss to my child/guardian or his or her property. This includes any loss arising as a result of personal injury, wrongful death, property damage and sickness(including with respect to COVID-19) which may arise as a result of any such participation in ENEH activities and matters related thereto. I waive any and all claims arising out of any of the foregoing including as a result of , or arising out of, any violation of any rules or policies with respect to COVID-19. All participants and/or parents acknowledge the COVID-19 pandemic and the various states and cities re-opening. We are following all guidelines as mandated, and by a member of your family participating in the course of instruction, acknowledges this and further asserts, that your family member has too been following all of the guidelines etc. In time(hopefully short) we will be over this situation.
By signing this agreement and waiver, it is also agreed that express permission is granted to Erik Nates Euro Hockey, Inc. to use photographs and videos in which the student’s image and likeness appear in connection with his/her participation in Erik Nates Euro Hockey, Inc. Said express permission shall apply to the display ,publication, distribution, use, print and reprint of such images and likeness, and the right to employ such images or likeness in advertising and promotions.
